then PO 1000 mg four times a day

Mild penicillin allergy

cefazolin

IV 2 g every eight hours

followed by early switch to

cefalexin

PO 500 mg four times a day

Severe penicillin allergy

clindamycin

PO 450 mg three times a day

Pathogens Known

S. aureus

S. pyogenes/other streptococci

benzylpenicillin

HMLSchedNZFPMLnoids

(Penicillin G)

IV 1.8 g (3 megaunits) every six hours

followed by early switch to

phenoxymethylpenicillin

(Penicillin V)

PO 500 mg four times a day

OR

amoxicillin

PO 500 mg three times a day
