Vancomycin Dosing Guidelines

We've updated this section:
- Initial vancomycin dosing is now based on actual body weight (loading dose) as well as renal function (maintenance dose).
- Evaluation of the area under the concentration-time curve (0 – 24 h), rather than trough concentration alone, is used for subsequent dose individualisation.
- Maximum infusion rate has increased to 1000 mg/hour.
